A guide to enabling the spread of person- and community-centred approaches for health and wellbeing

This guide is addressed to people who champion person- and community-centred approaches in health and social care. It is part of the NHS England-funded Realising the Value programme led by Nesta and the Health Foundation, which seeks to develop person- and community-centred approaches for health and wellbeing. Two main contents are included in the guide: a framework for understanding and changing behaviour, and real-world examples of how these changes happen in practice.
Change is more likely to spread when both statutory organisations and community-based organisations align approaches. For this reason the guide feature case studies from both parts of the system.  They are colour coded to enable different audiences to navigate the guide easily.

Meet Integrated Care Pioneers in Edinburgh, Manchester and Wrexham!

Study tours offer a unique opportunity to meet integrated care pioneers in their natural environment and to experience and learn about integrated care practices from national and regional innovations. They are designed to support clinicians, managers and policy-makers with an interest and responsibility in the successful adoption of integrated care in policy and practice. Designed to be live case studies, the study visits offer the possibility to discuss regional and country-level strategies with the responsible decision makers, assess the barriers, facilitators and key drivers for integrated care at a regional and local level and reflect upon the transferability of lessons learned in moderated discussions and workshops.

Progressing primary health care: a series of country case studies

The 1978 Declaration of Alma-Ata (1) was revolutionary. Many countries, the World Health Organization (WHO), the United Nations Children’s Fund (UNICEF) and other organizations had been pursuing strategies to provide primary health care (PHC) for years (2). However, Alma-Ata made PHC central to health care policy and practice. In a world in which resources had long been focused on “vertical” health programmes, expanding the focus of health care was a novel approach for the global health community. Since this declaration, efforts to improve health have increasingly been framed in terms of building, strengthening or realigning the systems that contribute to a more expansive notion of health for the entire population.
